We are committed to providing resources, best practices, networking opportunities, advancement tools and the latest information to our community.
Medicare refers to diabetes management support programs as Diabetes Self-Management Training (DSMT) and the accrediting bodies refer to the programs as Diabetes Self-Management Education and Support (DSMES). They are basically the same thing and acronyms can be used interchangeably. Diabetes Solutions is an accredited DSMES/DSMT program and provides a comprehensive curriculum to help you improve your blood sugars, feel your best, and reach your other health goals.
We do accept some health insurances for our Diabetes Solutions coaching services. We accept regular Medicare and Blue Cross Blue Shield. Unfortunately, Medicare Advantage does not provide reimbursement for DSMES/DSMT services. Regardless of your insurance status, we can work with you and would love the opportunity to help you better manage and combat diabetes and feel your best. And we can often to do so for very little to usually no out-of-pocket costs.
Scheduling a community health screening event is easy! Please visit our Contact page to get in touch with our team. We’ll guide you through the process and provide the necessary information to make your event a success.

Diabetes Solutions coaching services are available to people with any form of diabetes including prediabetes for very little to no, out of pocket costs. Sometimes participants may have a small out of pocket co-pay but if they are unable to afford the copay, we have grant funds available to cover their portion of the copay. We don’t want financial barriers to prevent anyone from getting the coaching or help they need to better manage diabetes. Please reach out to us if you have questions about whether Diabetes Solutions will cost you anything.
For people enrolled in our Diabetes Solutions coaching program, we can provide a Freestyle Libre continuous glucose monitor (CGM) for people with an A1c above 9.0% who otherwise could not afford to wear one. We can provide 2 sensors which will cover 28 days of wear.
You may qualify for the Freestyle Libre program to receive one 14-day CGM sensor for free. You can visit this website to determine if you qualify for a free sensor. You can request a prescription from your doctor who treats your diabetes and then fill the prescription wherever you get your prescriptions.
