Expanding screening policies, community events, and telehealth services to increase access to diagnosis, treatment, and management resources.

Offering online courses, utilizing former DSMT participants as CHWs, and providing support services to prevent and manage diabetes.

Training for the Diabetes Solutions model and offering educational opportunities to healthcare providers.
Reduce Healthcare Costs & Improve Patient Outcomes
Achieving target A1c levels, providing CGMs for remote patient monitoring, and reducing unnecessary healthcare utilization.
